PICOT QUESTION AND LITERATURE SEARCH

PICOT QUESTION AND LITERATURE SEARCH

PICOT Question and Literature Search
The first step of the evidence-based practice process is to evaluate a nursing practice environment to identify a nursing problem in the clinical
area. When a nursing problem is discovered, the nurse researcher develops a clinical guiding question to address that nursing practice problem.
For this assignment, you will create a clinical guiding question know as a PICOT question. The PICOT question must be relevant to a nursing
practice problem. To support your PICOT question, identify six supporting peer-revised research articles, as indicated below. The PICOT
question and six peer-reviewed research articles you choose will be utilized for subsequent assignments.
Use the “Literature Evaluation Table” to complete this assignment.
Select a nursing practice problem of interest to use as the focus of your research. Start with the patient population and identify a clinical problem
or issue that arises from the patient population. In 200–250 words, provide a summary of the clinical issue.
Following the PICOT format, write a PICOT question in your selected nursing practice problem area of interest. The PICOT question should be
applicable to your proposed capstone project (the project students must complete during their final course in the RN-BSN program of study).
The PICOT question will provide a framework for your capstone project.
Conduct a literature search to locate six research articles focused on your selected nursing practice problem of interest. This literature search
should include three quantitative and three qualitative peer-reviewed research articles to support your nursing practice problem.
Note: To assist in your search, remove the words qualitative and quantitative and include words that narrow or broaden your main topic. For
example: Search for diabetes and pediatric and dialysis. To determine what research design was used in the articles the search produced,
review the abstract and the methods section of the article. The author will provide a description of data collection using qualitative or quantitative
methods. Systematic Reviews, Literature Reviews, and Metanalysis articles are good resources and provide a strong level of evidence but are
not considered primary research articles. Therefore, they should not be included in this assignment.
While APA style is not required for the body of this assignment, solid academic writing is expected, and documentation of sources should be
presented using APA formatting guidelines, which can be found in the APA Style Guide, located in the Student Success Center.

Treatment Plan Assignment

Treatment Plan Assignment

Module 6: Treatment Plan Assignment

TREATMENT PLAN ASSIGNMENT: (Complete the Following Instructions)

For every person we see we must create a service or treatment plan that specifically addresses that person’s needs. In addition, this plan must specifically address the direction the individual states he/she wants to go. No two plans should be exactly alike because people can have the same problems but react to them differently.

When creating plans, always involve the client as well as the client’s family when appropriate. Plans without client input are not acceptable. As people discuss their problems and the issues they feel should be addressed, you can generate general goals for them that will become part of the plan. Once you have some ideas about goals, share your ideas with them and always ask for their opinions and additions.

The client represents an individual from case management with children and their families. Remember the ecological model, the child’s point of view, respect for children, prevention efforts and recognize children are one of our most vulnerable populations – careful attention must be paid to their protection.

For the following client, complete the following:

1) Create a new referral/inquiry sheet (note: you may need to create information such as address, phone, etc for the client). I’m attaching an example of what a referral sheet looks like, but in the end you can submit any referral form you have.

2) Create a list of items that will be included on the clients formal service plan.

3. Create a service/treatment plan and submit a document similar to the one in your textbook for your client and include the items in the instructions (again, you can use any treatment plan form you have within your own agency or one you find online, or simply follow the sample given in your textbook).

4. Make sure your submission has at least 450 words.

Assessing the Problem Leadership Collaboration Communication Change Management and Policy Considerations

Assessing the Problem Leadership Collaboration Communication Change Management and Policy Considerations

Assessment 1 Instructions: Assessing the Problem: Leadership, Collaboration, Communication, Change Management, and Policy Considerations

In a 5-7 page written assessment, define the patient, family, or population health problem that will be the focus of your capstone project. Assess the problem from a leadership, collaboration, communication, change management, and policy perspective. Plan to spend approximately 2 direct practicum hours meeting with a patient, family, or group of your choice to explore the problem and, if desired, consulting with subject matter and industry experts.

Nurses in all professional roles work to effect positive patient outcomes and improve organizational processes. Professional nurses are leaders in problem identification, planning, and strategy implementation—skills that directly affect patient care or organizational effectiveness.

Too often, change agents jump to a conclusion that an intervention will promote the envisioned improvement. Instead, the ideal approach is to determine which interventions are appropriate, based on an assessment and review of credible evidence. Interventions could be patient-facing or involve a change in policy and process. In this assessment, you’ll identify and make the case for your practicum focus area, then explore it in depth from a leadership, collaboration, communication, change management, and policy perspective.

This assessment lays the foundation for the work that will carry you through your capstone experience and guide the practicum hours needed to complete the work in this course. In addition, it will enable you to do the following:

  • Develop a problem statement for a patient, family, or population that’s relevant to your practice.
  • Begin building a body of evidence that will inform your approach to your practicum.
  • Focus on the influence of leadership, collaboration, communication, change management, and policy on the problem.

Demonstration of Proficiency

By successfully completing this assessment, you will demonstrate your proficiency in the course competencies through the following assessment scoring guide criteria:

  • Competency 1: Lead people and processes to improve patient, systems, and population outcomes.
    • Define a patient, family, or population health problem that’s relevant to personal and professional practice.
  • Competency 2: Make clinical and operational decisions based upon the best available evidence.
    • Analyze evidence from peer-reviewed literature and professional sources that describes and guides nursing actions related to a defined patient, family, or population problem.
  • Competency 5: Analyze the impact of health policy on quality and cost of care.
    • Explain how state board nursing practice standards and/or organizational or governmental policies could affect a defined patient, family, or population problem.
  • Competency 7: Implement patient-centered care to improve quality of care and the patient experience.
    • Propose leadership strategies to improve outcomes, patient-centered care, and the patient experience related to a defined patient, family, or population problem.
  • Competency 8: Integrate professional standards and values into practice.
    • Organize content so ideas flow logically with smooth transitions.
    • Apply APA style and formatting to scholarly writing.

Preparation

In this assessment, you’ll assess the patient, family, or population health problem that will be the focus of your capstone project. Plan to spend approximately 2 hours working with a patient, family, or group of your choice to explore the problem from a leadership, collaboration, communication, change management, and policy perspective. During this time, you may also choose to consult with subject matter and industry experts about the problem (for example, directors of quality or patient safety, nurse managers/directors, physicians, and epidemiologists).

To prepare for the assessment, complete the following:

  • Identify the patient, family, or group you want to work with during your practicum The patient you select can be a friend or a family member. You’ll work with this patient, family, or group throughout your capstone project, focusing on a specific health care problem.
  • Begin surveying the scholarly and professional literature to establish your evidence and research base, inform your assessment, and meet scholarly expectations for supporting evidence.

In addition, you may wish to complete the following:

  • Review the assessment instructions and scoring guide to ensure that you understand the work you’ll be asked to complete and how it will be assessed.
  • Review the Practicum Focus Sheet: Assessment 1 [PDF], which provides guidance for conducting this portion of your practicum.

Note: Remember that you can submit all, or a portion of, your draft assessment to Smarthinking Tutoring for feedback, before you submit the final version for this assessment. If you plan on using this free service, be mindful of the turnaround time of 24–48 hours for receiving feedback.

Create a change project for a new nurse preceptorship program based QSEN
teamwork/collaboration. The problem identified is staff nurses reluctance to precept nursing
students.
The change project must demonstrate the student’s ability to synthesize and apply the knowledge
and skills acquired in this academic program to real-world nursing issues and problems. This final
project will affirm one’s ability to think critically and creatively, to solve practical problems related to
nursing practice or the facilitation of patient-centered care, to make reasoned and ethical decisions,
to propose change and to demonstrate both oral and written communication effectively with peers,
nursing staff and members of the health care team.
Objectives:
Integrate Lewin’ s Change Theory in human systems when identifying the practice problem or issue
Propose an innovative solution or change to a system-related patient care problem identified on the
unit where the student is practicing.
Analyze the outcomes of nursing interventions using research-based evidence and data.
Formulate evidence-based conclusions and recommendations related to the problem identified.
Directions
Identify a practice/problem/policy on your assigned unit. Be specific as to how this affects the unit
you are working on.
Find evidence to support this practice/policy change. You may use research articles, meta-analysis

and integrative reviews from peer-reviewed journals.
Summarize the evidence from at least five professional sources. These sources should be organize
in a summary table.
You must create an appendix in your paper that includes: any surveys, the summary table of
sources, and any additional resources such as visual aids or tools created for this project.
Evaluate the evidence. Is the evidence strong enough to change practice/policy or support a curren
practice/policy?
Does the evidence support the practice/policy?
Make recommendations based on the evidence. How could you apply your recommendations on
your unit?
