COMMUNITY ASSESSMENT PAPER PROPOSAL
General Instructions:
Students will write a 3 – 4 page paper proposal that identifies and describes the community to be assessed in the community assessment paper. A demographic overview, map of the location designated and APA reference list is a part of the proposal. Map and reference list are not part of the page count.
Preliminary Reference List is required (2010 and later) for this assignment (Some references are possible to change if needed and or relevant:
A minimum of 6 references is required. Students will research a chosen topic within the community using professional empirically based knowledge obtained from professional journals, books, the course text, local community assessments such as the United Way, the CDD (Center for Disease Control), or other governmental organizations that provide professionally supported data.

Overview
A. Identify and discuss selected topic. Why interest in selected topic? (See community assessment paper rubric or syllabus for suggested topics/locations).
B. Identify and discuss location area. Why did you select this particular area?
C. Summary – Include reference to at least two of the six the core values of NASW code of ethics.
D. Location Map – Students may go to Google map or perhaps the interest site to obtain a map of the location area.
E. Reference list (APA) 2010 and later.

Evidence Based Practice and Applied Nursing Research

In APA format, the paper should be written following instructions provided.
The evaluator grades directly from the rubric and every part must be included.
Attached is a template for the paper as well as the rubric used for grading.

Effective Communication for Nursing

Nursing Communication
Effective communication is vital to constructing an accurate and detailed patient history. A
patient’s health or illness is influenced by many factors, including age, gender, ethnicity, and
environmental setting. As an advanced practice nurse, you must be aware of these factors and
tailor your communication techniques accordingly. Doing so will not only help you establish
rapport with your patients, but it will also enable you to more effectively gather the information
needed to assess your patients’ health risks.
For this Discussion, you will take on the role of a clinician who is building a health history for a
particular new patient assigned by your Instructor.
To prepare:
With the information presented in Chapter 1 of Ball et al. in mind, consider the following:
By Day 1 of this week, you will be assigned a new patient profile by your Instructor for this
Discussion. Note: Please see the “Course Announcements” section of the classroom for your
new patient profile assignment.
How would your communication and interview techniques for building a health history differ with
each patient?
How might you target your questions for building a health history based on the patient’s social
determinants of health?
What risk assessment instruments would be appropriate to use with each patient, or what
questions would you ask each patient to assess his or her health risks?
Identify any potential health-related risks based upon the patient’s age, gender, ethnicity, or
environmental setting that should be taken into consideration.
Select one of the risk assessment instruments presented in Chapter 1 or Chapter 5 of the
Seidel’s Guide to Physical Examination text, or another tool with which you are familiar, related to
your selected patient.
Develop at least five targeted questions you would ask your selected patient to assess his or her
health risks and begin building a health history.
By Day 3 of Week 1
Post a summary of the interview and a description of the communication techniques you would
use with your assigned patient. Explain why you would use these techniques. Identify the risk
assessment instrument you selected, and justify why it would be applicable to the selected
patient. Provide at least five targeted questions you would ask the patient.

Nursing Ethical Issues

Aim: This aim of this paper is to explore one ethical issue that you identified during your current
clinical placement and to develop strategies that would restore or enhance the ethics in the
situation and promote either autonomy or justice. You will use one of the seven value statements
from the CNA Code of Ethicsto help you identify and analyze the ethical issue. You will use the
peer reviewed scholarly literature to support your strategies. A secondary aim is to evaluate your
ethical reasoning so that you can describe how you would know that your plan has enhanced the
integration of the value statement and created a positive impact on patients or the ethical
community.
Paper Details:
This paper must use APA 6th edition formatting. The maximum length does not include the
references or title page. Please see the grading scheme for grades distribution. Should your paper be longer than six APA formatted pages, the additional pages will not be graded as part of
this assignment.
You are required to use three peer reviewed journal articles from the ethics literature. You cannot
use articles that were part of the syllabus. Additional (and necessary) sources such as the CNO
standards and references to support your theory do not count towards the required three peer
reviewed journal articles.

This is a 7 – 10 page powerpoint, articles must be summarized in a separate doc and speaker

Introduce the topic then, for each article, summarize the key points in no more than 2 paragraphs

summarize the implications these articles in current nursing practice (what affect does this topic
have on nursing).
Researching should be conducted in such data bases as CINHAL, MedLine etc.
Student will develop narrated PowerPoints and post on Blackboard. Each student will comment
on 2 student presentations.

Nursing Evidence-Based Practice and the Quadruple Aim
Healthcare organizations continually seek to optimize healthcare performance. For years, this
approach was a three-pronged one known as the Triple Aim, with efforts focused on improved
population health, enhanced patient experience, and lower healthcare costs.
More recently, this approach has evolved to a Quadruple Aim by including a focus on improving
the work life of healthcare providers. Each of these measures are impacted by decisions made at
the organizational level, and organizations have increasingly turned to EBP to inform and justify these decisions.
To Prepare:
Read the articles by Sikka, Morath, & Leape (2015); Crabtree, Brennan, Davis, & Coyle (2016);
and Kim et al. (2016) provided in the Resources.
Reflect on how EBP might impact (or not impact) the Quadruple Aim in healthcare.
Consider the impact that EBP may have on factors impacting these quadruple aim elements,
such as preventable medical errors or healthcare delivery.
To Complete:
Write a brief analysis (no longer than 2 pages) of the connection between EBP and the
Quadruple Aim.
Your analysis should address how EBP might (or might not) help reach the Quadruple Aim,
including each of the four measures of:
